The locking wire is too loose or the locking wire is disconnected.

Tighten or reconnect the locking wire in order to allow the lock bar to disengage from the locking slots in the adjuster lower channel when the lock control lever is activated. Refer to Front Seat Adjuster Replacement

The adjuster lock bar sticks or binds.
