INCORRECT COMPLETION OF AC-DELCO FORM ACD1078

A recently installed information audit and verification system at AC-Delco has resulted in an extremely high percentage of incorrect vehicle identification numbers as well as other vital information due to incomplete and/or illegible handwritten entries on the ACD 1078 warranty repair claim form.

All information contained in this form shares equal importance due to its varied applications related to product reporting by both AC-Delco and Buick.

To insure the receipt of accurate information and avoid the possibility of rejected AC-Delco warranty claims, the following two items must be complied with:

1. "Verify" all information before and after making entries on the ACD 1078 form.

2. Follow "All" instructions printed on the ACD 1078 form concerning proper completion of the form which include, in bold print, "DEALERS COMPLETE ALL RED SHADED AREAS - MUST BE TYPEWRITTEN."

Please distribute this bulletin to all effected dealership personnel and stress the importance of its content, ACCURATE TYPEWRITTEN ACD 1078 Forms.
