This supplement is being issued in order to advise that the template initially provided for use with the "Service Procedure" of Campaign Bulletin 82-C-16, issued December, 1982, is incorrect.

Revised templates are attached to this supplemental bulletin. Destroy all templates sent with the original bulletin and use the attached revised templates in their place. Continue to use all instructions as stated in Campaign Bulletin 82-C-16.
