TO ORDER EXCHANGE INJECTION PUMPS FOR AFFECTED VEHICLES, THE MD- TICS EXCHANGE PUMP REQUEST FORM (TSVCF-5128) MUST BE COMPLETED. THIS FORM HAS RECENTLY BEEN UPDATED (REVISION DATE 3/94).
FOLLOW THE INSTRUCTIONS IN THIS SERVICE BULLETIN FOR COMPLETING THE UPDATED MD-TICS EXCHANGE PUMP REQUEST FORM. TEN COPIES OF THE FORM ARE ENCLOSED WITH THIS SERVICE BULLETIN.

COMPLETING DEALER SECTION OF FORM:
1. REPAIR TYPE. CHECK THE APPROPRIATE BOX FOR ONE OF THE FOLLOWING:
- VEHICLE DOWN: INJECTION PUMP FAILURE, VEHICLE IS NON-OPERATIONAL. - CAMPAIGN VEHICLE: INJECTION PUMP REQUIRED TO COMPLETE CAMPAIGN.
2. VEHICLE STATUS. CHECK THE APPROPRIATE BOX FOR ONE OF THE FOLLOWING:
- RETAILED/LEASED VEHICLE: VEHICLE IS CURRENTLY BEING USED AND NOT IN DEALER INVENTORY - DEALER INVENTORY VEHICLE: VEHICLE IS IN DEALER INVENTORY AND HAS NOT BEEN SOLD
3. DEALER TYPE. CHECK THE APPROPRIATE BOX FOR ONE OF THE FOLLOWING:
- CHEVROLET TRUCK DEALER - GMC TRUCK DEALER
4. DEALER INFORMATION. WRITE IN THE DEALER'S NAME, DEALER CODE, SHIPPING ADDRESS (ZIP CODE REQUIRED), DEALER CONTACT NAME, PHONE NUMBER AND FAX NUMBER.
5. VEHICLE INFORMATION. WRITE IN THE FOLLOWING INFORMATION:
- VIN: VEHICLE IDENTIFICATION NUMBER (17 DIGITS). - TRANSMISSION: VEHICLE TRANSMISSION TYPE. ENTER M/T IF MANUAL; A/T IF AUTOMATIC. - IN-SERVICE DATE: DATE THE VEHICLE WAS PUT INTO SERVICE. - MILEAGE: VEHICLE'S MILEAGE AT THE TIME OF REPAIR - CUSTOMER NAME: NAME OF VEHICLE OWNER
6. REPAIR INFORMATION. WRITE IN THE FOLLOWING INFORMATION:
- CUSTOMER COMPLAINT: DESCRIBE THE PROBLEM, I.E., LOSS OF POWER. IF THIS IS A CAMPAIGN REPAIR, ENTER "CAMPAIGN". - FAILED INJECTION PUMP ASSEMBLY #: 4-DIGIT NUMBER ON THE INJECTION PUMP TAG. - INJECTION PUMP SERIAL #: 10-DIGIT SERIAL NUMBER ON THE INJECTION PUMP TAG (THREE NUMBERS, ONE LETTER, FOLLOWED BY SIX NUMBERS).
7. REPAIR INFORMATION FOR VEHICLE DOWN UNITS. WRITE IN THE FOLLOWING INFORMATION:
- TYPE OF VEHICLE OPERATION: LIST PERCENTAGE (%) OF CITY DRIVING, HIGHWAY DRIVING, HILLY ROADS DRIVEN AND IDLING DURING NORMAL VEHICLE OPERATION. - WATER SEPARATOR MOISTURE: IS WATER PRESENT IN THE WATER SEPARATOR? (WRITE "YES" OR "NO"). IF YES, INDICATE THE PERCENTAGE (%). - DIAGNOSIS: WAS A SCAN TOOL USED TO DIAGNOSE THE VEHICLE (WRITE "YES" OR "NO")? IF A SCAN TOOL WAS USED, WRITE A CHECK MARK IN THE APPROPRIATE TROUBLE CODE BOX(ES). IF A SCAN TOOL WAS NOT USED, LIST THE "CHECK ENGINE" LIGHT CODES WHICH WERE FLASHED, IF ANY.
