The wire assembly is too loose or disconnected.

Eliminate the slack in the wire assembly by moving the hooked end to the rearward hole of the lock bar.

The lock bar is sticking or binding.

Lubricate the lock bar pivot. If the bar is binding, eliminate the cause of the binding or replace the adjuster.
