WIRING FIELD SURVEY FORM UPDATE

In an ongoing effort to improve the product quality of our vehicles, the Wiring Field Survey form has been updated. Please complete this form whenever electrical work is performed on an in-warranty vehicle. Please include the following information:

Complete 17 digit VIN, Date, and Mileage

Dealer Code

Description of Customer Complaint

Complete Description of What Caused the Condition

In the upper right hand corner of the form, write in the labor op used (see attached form).

The new form has two sides. One is for Fiero only and the other for all other cars. Completed forms should be sent to:

Dealers with planning potential over 300 would have received a new pad with the original bulletin; please discard the old forms. Dealers under 300 should use the old forms, and reorder the new form when necessary on the Dealer Stationary Supply Order Form.
